Application Form
To apply, please fill out the information below. Applications must be submitted with a $100 non-refundable application fee in order for your application to be considered and processed. Please review what supporting documents that are required for your course. This can be found on the courses page on the downloadable pdf for each course. Offers of admission for USS and Occ. Scuba will be given once the application and fee, transcript, resume and interview are complete. All remaining required documentation must be submitted prior to starting the course.
